I, ANN CHIEJINE, wishes to inform the general public and all relevant public and private organisations that my official name is and remains ANN CHIEJINE.

I hereby declare publicly that the inclusion of ONYEKA as a middle name in any of my records should be voided. All my official documents including my Nigerian international passport and National Identification Number (NIN) registration bear my official name of ANN CHIEJINE.

I wish to maintain my official identification consistently as ANN CHIEJINE.
